NOTE:  Enrollment fees do not include food.  Please pack snacks and a lunch.  Fees are prorated for short weeks involving a holiday, such as 4th of July.

In Case of Emergency Form

General Permission Form

The "In Case of Emergency" form allows Columbia Academy to consent to medical treatment for your child in the event of an emergency.  The "General Permission" form allows your child to participate in activities such as field trips and swimming, and allows us to share photos of the fun with you via our online Photo Gallery (email Michele for your password).

 

Your completed paperwork may be hand-delivered, faxed to (503) 699-1013 or mailed to P.O. Box 101, West Linn, Oregon 97068.  A deposit may be required to reserve your child's space (determined based on length of enrollment).
